Giving patients the ability to pre-register for an operation has endless time-saving implementations both for the patient and the company. This form includes some of the most general questions encountered in situations that can use pre-registration. It includes Procedural Questions, Clinical Predictors of Cardiovascular Risk, Clinical predictors of Sleep Apnea, and other basic history questions to verify former surgeries and allergies to drugs.
