Materials Tracking/Request
INTRODUCTION: This form allows employees to request a wide range of materials necessary for daily business operation. The workflow then automatically sends the completed request to the appropriate department for authorization and delivery.

DESCRIPTION: This customizable form allows anyone with access to Teaming to submit a detailed request for materials. The request is made under one of ten customizable categories: Hardware, Furniture, Appliances, Office Equipment, Office Supplies, Vehicles, Tools, Chemicals, Machinery, and Other. Selecting one of these categories allows the form to be sent automatically to the correct department or office. The remainder of the form allows the user to select specific materials from a list under each main category, and also allows for a detailed description of the materials needed. The form includes space for the date and time of the request for ease of tracking.
Upon submission, the request is received by the appropriate department for review. The reviewer is automatically prompted to input his or her response, which includes reviewer name, reviewer response (approved or denied) and any reviewer comments regarding the response. When the review is complete, the reviewer then sends the request to the next state (Materials in Delivery or Rejected, depending on the response). If the request is sent to Rejected, the workflow is terminated. If the request is approved, and the applicant has received the materials, the form is sent to the final state (Materials Delivered).
This workflow can be customized to allow the request submitter to watch the status of the request at all times in the process, in order to see whether the request has been approved or denied, and whether the materials are in delivery or have already been delivered.
