Facilities Issue Report


INTRODUCTION: This form allows employees to report any problems with company facilities. The workflow then automatically submits the report to the appropriate department within the company for resolution.

DESCRIPTION: This easily customizable form allows anyone with access to Teaming to report facilities problems in one of six initial categories: Security, Maintenance, Custodial, Grounds, IT, and Other. The list can be modified or extended to match any corporate structure or unique facility need. The initial selection of the category determines the path of the workflow when the report is submitted.

The form includes drop-down areas associated with each of the six original categories to allow for the selection of very specific facility issues ( leaky faucet, car with lights left on, heating/air conditioning problem, etc...). The form then allows the employee to enter specific information detailing the issue, such as location and severity. The employee can also include the date and time of the report and his or her name.

Upon submission, the workflow automatically reads the data entered into the form to determine which department is best able to resolve the issue. The report is then sent to that department for resolution. If the issue is not resolved within a certain (customizable) time period, the workflow then sends the report to the facilities manager, who can then personally task the appropriate department in order to ensure prompt issue resolution.
